A Book Plate will appear on the inside cover of each book you donate with your name and the name of the person honored or remembered. All books donated using this form will have the same dedication. If you wish to make multiple donations with different dedications, please use separate forms for each dedication. So that the person(s) honored (or surviving relative of the person remembered) may receive a letter informing them of your donation, the following form will request their address.
