CPCN Family Practice Summit 2024 Volunteer/Speaker Registration

Join us for the 2024 Chinook PCN Family Practice Summit


Our focus for this year's summit is Together for Y(Our) Health.  We know how important it is to understand the community and patients we serve. The health of the community is reflected in the individuals who live there, and the health of the individual is reflected back in the community. We also know clinic teams need to be healthy to properly serve the patients and communities as well.  That is why this year we are excited to bring together different speakers and topics which reflect our community, our teams, and the system. 

This year's morning includes two breakout areas.  Breakout A is a poverty simulation, where you get to live in the shoes of a person living in poverty, trying to understand and navigate a complex system.  The Community Action Poverty Simulation is a tool that helps participants rethink the challenges that millions of low-income individuals must face each and every day. More importantly, this tool helps people identify areas of change that can directly impact the effects of poverty on individuals, families, and communities.

In Breakout B, you will watch "Falling Through the Crack's" The Greg Price story. In this breakout session facilitated by Dave and Terri Price, we go beyond the film to network and discuss how healthy teams can work together to close gaps in their clinic system.  Following the screening, Dave and Terri Price will lead an interactive conversation, using teaching scenes, to help teams understand what areas of the system they can influence to close the gaps.  

Lunch will be provided and is a time to network with your peers and checkout the amazing program booths that will be set up, including the AMA, ACTT, CII CPAR, FAST and many more.  

The afternoon will be based on information gathering and sharing for clinic physicians and their teams.  

We will be bringing together physician leads from AHS, the South Zone Physician Lead Executive and AMA for a panel discussion and question period where physicians and staff can ask for clarification and updates directly from those who are closest to it.  

We will also have a few specialty practices presenting clinical updates in their areas of work. 

Lastly, we are very excited to have Chris Koch round off the day as our Keynote Speaker with his "If I Can" presentation.  

This is a packed day with something for everyone.  We are excited to see you and your team.
