Five Wishes is a living will that allows you to talk about your personal, emotional, spiritual, and medical wishes. It will enable you to choose a person you want to make health care decisions for you if you are not able to make them for yourself. It lets you say how you wish to be treated if you get seriously ill. It enables you to have this conversation with your family before you become seriously ill and are in a crisis mode. This easy to use document (which you will receive at the seminar) will cover the following five wishes: <ol fr-original-style="" style="width: 100%; box-sizing: inherit;"><li fr-original-style="" style="box-sizing: inherit;">The person I wish to make care decision when I can&rsquo;t</li><li fr-original-style="" style="box-sizing: inherit;">The type of medical treatment I wish to receive</li><li fr-original-style="" style="box-sizing: inherit;">How comfortable I want to be</li><li fr-original-style="" style="box-sizing: inherit;">How I want people to treat me</li><li fr-original-style="" style="box-sizing: inherit;">What I want my loved ones to know</li></ol>Five Wishes is for anyone 18 or older &ndash; married, single, parents, adult children and friends. People who use Five Wishes find it helps them express all that they want and provides a helpful guide to family members, caregivers and medical staff.
