2025 THS Parent Contract

The parents of our players have a significant role in the success and growth of our program. By E-signing below, you are agreeing to uphold the following standards and rules:----

Make sure your child attends all practices and games. Inform the Head Coach about any missed practices and games before they occur. Your player will receive sanctions for not following this rule.----

Encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every practice or game.----

Place the emotional and physical well being of your child ahead of any personal desire to win. A single game is not worth damaging the self esteem of your player.----

Report any significant injuries immediately to the Head Coach.-----

Remember that this organization is for young players to improve their skills, not for adults to live out their dreams through their children.----

Do your best to assist the team with all associated activities, car pools, fundraisers, etc.----

Allow 24 hours to pass from the END of a tournament to discuss any negative issues with your coach. This allows a cooling down period and does not interfere with the coach’s ability to prepare for the next game during a tournament.----

Informed Consent/General Release-Girls Basketball Participants

PLEASE READ CAREFULLY AND SIGN BELOW TO INDICATE YOUR AGREEMENT.

NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.

Since participation in youth sports activities can be dangerous, THS requires that all participants (and their adult parent(s) or guardians) to assume all risks associated with youth basketball by signing this general release.

I, the parent and/or guardian of the above-named Registrant, do hereby give my approval for the Registrant to participate in any and all activities provided by THS. I certify that the Registrant is in good physical condition and is fit to participate in all THS activities and suffers from no physical impairment that might be aggravated by any such participation.

I certify that the Registrant is covered by medical insurance, and that I will not hold THS, its officers, directors, managers, or coaches, or any other representatives responsible for any injuries or harm to Registrant which occurs during participation in any practices, tournaments and or any related activities to THS.

I AGREE TO THE FOLLOWING BELOW:

1. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, potentially life-threatening, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I acknowledge that I am aware that there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from:

• An outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID19) and/or any mutation or variation thereof;

5. In consideration of having the opportunity to participate as either a team member or competitor at location, and in acknowledging that I am aware of and willing to assume the risks associated with this activity, I hereby voluntarily agree to waive, hold harmless and indemnify THS. And its trustees, agents, volunteers and

Employees from any and all claims, demands, damages and causes of action of any nature whatsoever arising out of ordinary negligence which I, my heirs, my assigns or successors may have against them for, on account of, or by reason of my participation in the above activities. I indicate my agreement to this hold harmless elective noted below.

Recognizing, the possibility of physical injury associated with various activities and in consideration for THS, accepting the Registrant for it’s the tours related activities, I HEREBY RELEASE, DISCHARGE, WAIVE, ABSOLVE, INDEMNIFY, AND AGREE TO HOLD HARMLESS THS, its officers, directors, coaches, team managers, representatives and other associated personnel, including the owners of the all activities the group utilizes for THS, against any and all claims and causes of action, including negligent acts, as a result of the Registrant’s participation with THS that may result in injury or damage to the Registrant as a result of the Registrant’s participation in Programs.

As the parent/guardian of Registrant, I further hereby give my consent to a coach, team manager, administrator, or other THS representative to authorize the administration of any reasonable and necessary medical or dental care to the Registrant and further agree to be financially responsible for the costs incurred for such care.

I understand that departure from the program ahead of its conclusion does not entitle me to a refund of fees.

I agree that I and the Registrant will abide by all the rules, policies and procedures of THS, or any other applicable affiliated organizations or sponsors. I further understand that the failure to comply with any such rules, policies, and procedures may result in Registrant being subject to disciplinary action, which may include expulsion from THS. I further agree that as a result of any such expulsion, suspension or disciplinary action, I will not be entitled to refund of any previously paid Fees.

CONSENT FOR MEDICAL TREATMENT (MINOR)

As the parent or legal guardian of the above-named player, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being of my dependent.

MEDIA CONSENT FORM AND RELEASE

I am the parent/guardian of (“My Child”). I hereby grant The Hoop Society (“THS”), and their agents the absolute right and permission to use photographic portraits, pictures, digital images or videotapes of My Child, or in which My Child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any Social Media or on the teams websites, without payment or any other consideration. I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein My Child’s likeness appears, or the use to which it may be applied. I hereby release, discharge, and agree to indemnify and hold harmless to THS and their agents from all claims, demands, and causes of action that I or My Child have or may have by reason of this authorization or use of My Child’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, or any other advertisements or promotional materials. I represent that I am at least eighteen (18) years of age and am fully competent to E-sign this Release.

I have read this Informed Consent/General Release, fully understanding its terms, that I give up substantial rights by signing it, and sign it voluntarily.

2025 THS Payment Contract

I understand that my daughter will NOT be allowed to practice with the team or play in games/tournaments until payment $500 is made. 


I understand that all payments must be made ONLINE by the due date or paid in Cash to the Head Coach by the due date.

I understand that if I do not make all payments for the 2025 season that I am still liable for the balance and must pay that in full before the beginning of the next season to be eligible for the 2026 season. Payments will only be accepted ONLINE for delinquent balances.
