Affordable transportation for Baltimore and Prince George&#39;s County MD area HU students for Thanksgiving break Please complete this form to register for bus transportation for Thanksgiving break and remit the non-refundable $25 deposit by October 16th &nbsp;&nbsp;to secure your seat. Final payment is due November 6th. Due to the unpredictability of traffic, please be sure to provide a contact number that can be used to update you on the bus&#39;s arrival at applicable stops, if necessary. The coach bus is equipped with a bathroom and wi-fi. There is capacity for the student&#39;s luggage needs The Baltimore Metro NHAA is not responsible for lateness at either end. Consent and Authorization:By completing the registration, I, the undersigned/buyer, [Responsible Adult age 18+], hereby grant permission for the named student to participate in the upcoming bus trip to and from Hampton University scheduled to take place on November 24, 2026 and returning November 29, 2026.Health Information: I understand that the chaperones will take necessary precautions to ensure the safety and well-being of the students during the trip.&nbsp;Emergency Medical Treatment: In the event of an emergency, I authorize the chaperones to seek medical treatment for the student. I understand that every effort will be made to contact the listed me and the listed emergency contact in case of a medical emergency.Supervision and Behavior: I acknowledge that the student will be under the supervision of alumni chaperones during the trip. I expect the student to adhere to the code of conduct and behavioral expectations.Communication During the Trip: I will provide reachable contact numbers during the trip, and I understand that the chaperones may contact me in case of any important updates or emergencies.Transportation: I acknowledge that the student will travel via coach bus and is expected to adhere to the meeting time schedules. Baltimore NHAA is not responsible for lateness at either end.Cost and Payment: I understand the cost associated with the trip, which includes transportation and snacks. I agree to make the $25 non-refundable deposit by 10/16/26 and final $135 payment by 11/6/26. Cancellations received on or before October 24, 2026 will receive a refund, less the $25 deposit. Cancellations after October 24, 2026 will not be refunded. If I do not make final payments by 11:59 pm ET on November 8, 2026, my reservation will be considered a cancellation.Photography and Media Release: I grant permission for the student&#39;s image or video to be taken and used by the Baltimore NHAA for promotional or educational purposes.
