CAMSA On The Move (COTM) brings together community members and healthcare professionals to support stronger health in the Caribbean American community. By creating your own fundraising page, you help expand health education, screenings, and science-based programs. Your effort fuels workshops, research collaboration, and outreach that address real health disparities. Start a team, move in the way that fits you, and invite others to give. Every step you take helps move our community toward better care and better knowledge. COTM is a six-day hybrid wellness and fundraising challenge held during Caribbean Wellness Week. Participants complete physical activities from anywhere in the world while raising funds for CAMSA. The week concludes with an in-person celebration on September 19th, 2026, including a community walk/hike, networking, sponsor recognition, and awards.

CAMSA ON THE MOVE 2026 PARTICIPANT WAIVER, ASSUMPTION OF RISK AND RELEASE  Event Dates: September 14–19, 2026  CAMSA On The Move is a virtual and in-person wellness challenge that may include walking, running, cycling, swimming, dancing, hiking, fitness activities and related community events. The challenge concludes with an in-person community walk/hike at Shirley Chisholm State Park.  By registering for or participating in CAMSA On The Move, I acknowledge and agree to the following:  1. Voluntary Participation  I understand that my participation is voluntary. I am responsible for selecting activities that are appropriate for my health, physical condition, experience and ability.  2. Acknowledgment and Assumption of Risk  I understand that physical activity involves inherent and unforeseen risks. These may include, but are not limited to, falls; collisions; uneven terrain; traffic; weather conditions; heat or cold exposure; dehydration; overexertion; muscle or joint injuries; illness; equipment failure; contact with other people, animals or objects; and serious injury, disability or death.  I knowingly and voluntarily accept and assume the risks associated with my participation, whether I participate independently, virtually or at an activity organized in connection with CAMSA On The Move.  3. Health and Medical Responsibility  I confirm that I am physically able to participate or have consulted a qualified healthcare professional if I have concerns about my health or ability to participate.  I understand that CAMSA does not provide medical advice, medical screening or individualized supervision. I agree to stop participating and seek appropriate medical assistance if I experience pain, dizziness, shortness of breath, weakness or other concerning symptoms.  I am responsible for my medications, hydration, nutrition, protective equipment and any medical devices I may require.  4. Safety Responsibilities  I agree to exercise reasonable care and follow all applicable laws, posted rules, traffic regulations, park requirements and safety instructions. I will use appropriate clothing and equipment, remain aware of my surroundings and avoid participating under conditions that I believe are unsafe.  For self-directed and virtual activities, I am responsible for choosing my location, route, activity, equipment and level of exertion. I understand that recording or submitting an activity through Strava or another platform does not mean that CAMSA has inspected, approved or supervised my route or activity.  5. Emergency Medical Care  If I require emergency assistance during an in-person CAMSA On The Move activity and I am unable to provide consent, I authorize event representatives to contact emergency services and arrange reasonably necessary assistance.  I understand that CAMSA does not guarantee the availability of medical personnel and that I am responsible for medical, ambulance, transportation or other expenses incurred on my behalf.  6. Release of Claims  To the fullest extent permitted by applicable law, I release and hold harmless CAMSA and its officers, directors, members, employees, volunteers, representatives, sponsors, partners and agents from claims arising from the inherent risks of my voluntary participation in CAMSA On The Move.  This release is not intended to waive claims that cannot lawfully be waived, including claims arising from gross negligence, reckless conduct or intentional misconduct.  7. Personal Property  I am responsible for my personal belongings and equipment. CAMSA is not responsible for property that is lost, stolen or damaged during participation, except where responsibility cannot legally be excluded.  8. Third-Party Platforms and Locations  I understand that CAMSA On The Move may use third-party services and locations, including Zeffy, Strava and public parks. My use of those services and locations may also be governed by their respective terms, privacy policies and rules. CAMSA is not responsible for the operation, availability or data practices of third-party platforms.  9. Electronic Acceptance  By checking the required box during registration, I confirm that:  • I have carefully read and understood this waiver. • I have had the opportunity to ask questions before participating. • I voluntarily accept its terms. • I am at least 18 years old and legally able to provide this consent. • The electronic acceptance has the same effect as my signature.
