We are thrilled that you will be joining us for our celebratory grateful gatherings this year! This ticket form is for you to submit your location's total meal quantities for pick-up at the date, time and location listed above, under the header of your location. All orders need to be placed one week prior to your pick-up date.
Completing the form
Before you get started, please verify that your location is correct in the header. If it is, you can begin by simply selecting the "1" in the drop down menu then by pressing the "select" button. After that you will be prompted to provide your information and your location's total beef, chicken and vegan meal quantities. Nutrition services will be reviewing all orders submitted for accurate headcounts so only order for the people working at your location.
Pick-up
Once you complete your form, you will be emailed a QR code. Please bring that QR code with you, printed or on your phone, when picking up your order. Nutrition services will need to scan the code when you arrive to confirm your order was received.
