Putnam General Hospital Foundation - 2024 RAFFLE!


Putnam General Hospital (PGH) is committed to providing local access to high quality care. Using equipment consisting of the latest technological upgrades, the hospital provides valuable services to the community, including cardiac rehabilitation, emergency care, laboratory, radiology/imaging, therapy, swing bed and surgery. 


In support of its mission, the PGH Foundation works with the hospital to determine needed improvements in equipment or services and then assists the hospital by raising funds to secure these improvements intended to better serve you and others in the community.  


Spring/Summer Fundraiser: We are hosting a RAFFLE for a $500 Cash Prize! (See below for Raffle Rules & Regulations.) The winner will be announced at the Putnam County Dairy Festival on June 1, 2024. Tickets are 1 for $5 or 5 for $20.


We are also accepting donations, which can be made here on this app, or through the Georgia Heart Tax program. (Visit www.georgiaheart.org to learn more about this tax credit that benefits our rural Georgia hospital.)

RAFFLE RULES AND REGULATIONS

This raffle is conducted by Putnam General Hospital Foundation (the “Foundation”), located at 101 Lake Oconee Parkway, Eatonton, Georgia 31024. The Foundation is a non-profit organization under Internal Revenue Code Section 501(c)(3). All proceeds, less expenses, will benefit Putnam General Hospital. 

The raffle will be conducted from April 18, 2024 8:00 a.m. to June 1, 2024 with tickets available for purchase through 2:00 p.m. on June 1, 2024 Eastern Time. 

Tickets will be sold at $5.00 per ticket or $20.00 for 5 tickets. The odds of winning are dependent on the number of valid entries.

Purchasers of tickets must provide their name, phone number and email address (if applicable) on the Ticket Purchase Roster or the Online Submission Form at time of purchase.

If more than one name is provided per ticket, the first name listed on the Roster or Form shall be considered the “entrant.” The Foundation shall, will and can have no obligation for delivery of any prize to any person other than the entrant.

Minors may not purchase or have their names identified as entrants for prizes. Payment for the tickets shall be made in “good money” which may be cash, check or credit (if purchasing online).  Checks should be made payable to “Putnam General Hospital Foundation.” If any purchaser’s check is dishonored by the bank, the raffle ticket will be declared invalid unless and until full payment in “good money” is made prior to the drawing.  All purchases are final and no refunds will be issued.

The winner will be picked by public random drawing to be held at 2:15 p.m. on June 1, 2024 at the Putnam General Hospital Foundation booth at the Putnam County Dairy Festival. 

The winner need not be present to win. The winner will be contacted immediately after the drawing.

The winner agrees to allow use of their name or likeness for publicity purposes without compensation.

The raffle prize will be $500 Cash. 

The winner must present the raffle ticket when claiming their prize and provide one form of picture identification. Delivery of the prize shall be made solely to the entrant based on providing the raffle ticket stub and picture identification. 

The winner is subject to all Federal and State tax regulations and payment of any taxes is the sole responsibility of each winner. The Foundation will file a report pertaining to the prize to the Putnam County Sheriff on or before April 15, 2025 in accordance with Georgia Code § 16-12-22.1.

If the winner cannot be reached at the phone number, email address or mailing address on the ticket stub for any reason by June 20, 2024, a disqualification will result, and an alternate winner will be drawn.

A copy of these rules and eligibility requirements will be posted at Putnam General Hospital in the reception area.

The employees of the Hospital, as well as Foundation board members, at their discretion, may participate in the raffle.

The decision of the Foundation President shall be final on all matters concerning the raffle.
