This form is to notify the Memorial Service Program Committee of the death of a Lion in 2026 and ensure that he/she is remembered in the upcoming service.

1. Email or mail a high quality color photo. Digital photps shoud be 300 dpi/ppi in size.

2. Include 3 or 4 bullet points about the departed member.

3. Designate someone from you club or a family member to participate in the candle lighting ceremony. The person must meet us 30 minutes prior to the commencement of the service for instructions.
