As the person, parent, or guardian of the mentioned participant(s), I, hereby give my permission for the said participant(s), to attend LIFECHURCH7 Youth Group, aka ENCOUNTER YOUTH, SILVERWOOD TRIP. In the event of an emergency affecting the health or welfare of my child/student/participant, I give permission to the Youth Pastors, Youth Leaders, or Adult Chaperones to administer first aid and/or transport my child/student/participant to the nearest doctor or medical facility for further medical attention as deemed necessary. This includes but is not in limitation to emergency surgery or medical treatment. I also acknowledge that this individual action in response to the emergency will be held blameless and I assume all responsibility for any medical expenses that could occur due to my child's/student/participant(s) medical emergency; this would also include any traveling costs should my child/student/Participant need to return home early due to medical reasons or even disciplinary action. I ALSO acknowledge that my child will be riding in church vehicles to a public event.
