Community Health Worker (CHW)

Valid until October 8, 2027

A community health worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Individual Membership (Ally)
$40

Valid until October 8, 2027

An individual who believe in the power of Community Health Workers (CHWs) and wants to support a stronger, more connected health and community care system. Ally Members include healthcare professionals, educators, public health advocates, retirees, and community supporters who share our mission.

Community Based Organization (Nonprofit)
$150

Valid until October 8, 2027

For organizations that employ two or more Community Health Workers (CHWs) or actively support CHW programs and community-based care. Organizational Members partner with the Florida Community Health Worker Coalition to strengthen the CHW workforce through collaboration, professional development, advocacy, and sustainable community health initiatives while demonstrating their commitment to improving health outcomes across Florida. 

For Profit Organizations

Valid until October 8, 2027

For businesses and private-sector organizations committed to advancing Community Health Workers (CHWs) and community-centered care. Connect with a statewide network of leaders, support workforce development, collaborate on innovative solutions, and demonstrate your organization's commitment to healthier communities across Florida.

Health Systems and Institutions of Higher Education
$250

Valid until October 8, 2027

For hospitals, health systems, FQHCs, RHCs, colleges, universities, and academic institutions that employ, educate, train, or support Community Health Workers (CHWs). Join a statewide cross-sector collaborative dedicated to advancing workforce development, innovative care models, research, and sustainable community-centered care throughout Florida.
