Liability Waiver, Release & Assumption of Risk Agreement

Event: Seventh Day Slumber — Live in Concert (free admission)

Please read this agreement carefully. By registering for and/or attending this event, you ("Attendee," "I," or "me") agree to all of the terms below on behalf of yourself and any minors or guests in your party.

1. Voluntary Participation

I understand that admission to this concert is free and that my attendance is entirely voluntary. No one is requiring me or anyone in my party to attend.

2. Assumption of Risk

I understand that a live concert involves inherent risks, and I knowingly and voluntarily accept them. These risks include, but are not limited to:

• Exposure to loud, amplified music and the potential for temporary or permanent effects on hearing;
• Stage and crowd lighting effects, which may include bright, colored, moving, or flashing lights;
• Large crowds, standing, and crowd movement, including the possibility of bumping, pushing, or contact with other attendees;
• Slips, trips, and falls on stairs, cords, flooring, parking areas, or uneven ground;
• General risks associated with gathering in a public event space, including the actions of other attendees.

I understand it is my responsibility to take reasonable precautions for my own safety and the safety of anyone in my care, including using hearing protection if I choose, supervising my children at all times, and following the instructions of event staff and volunteers.

3. Release & Waiver of Claims

In exchange for being permitted to attend, I release, waive, and discharge the Released Parties (defined below) from any and all claims, demands, liabilities, or causes of action for any injury, illness, death, or loss of or damage to property that I or anyone in my party may suffer in connection with attending this event, including claims arising from the ordinary negligence of the Released Parties, to the fullest extent permitted by law.

"Released Parties" means: Hiwasse First Baptist Church and its pastors, elders, staff, employees, volunteers, members, and affiliated ministries and groups; Seventh Day Slumber and its members, management, agents, crew, and representatives; and the event's sponsors, vendors, contractors, and the owner of the property where the event is held.

4. Indemnification (Hold Harmless)

I agree to indemnify and hold harmless the Released Parties from any claims, losses, damages, or expenses (including reasonable attorney's fees) brought by me, by anyone in my party, or by anyone acting on our behalf, arising out of or related to our attendance at this event.

5. Personal Property

I understand the Released Parties are not responsible for any personal property that is lost, stolen, or damaged before, during, or after the event. I am responsible for my own belongings.

6. Emergency Medical Care

In the event of an injury or medical emergency, I authorize event staff to seek emergency medical assistance on my behalf or on behalf of a minor in my care. I understand and agree that I am responsible for any resulting medical costs.

7. Photo, Video & Media Release

I understand this event may be photographed, filmed, livestreamed, or recorded. I grant the Released Parties permission to use my likeness (and that of minors in my care) in photos, video, and audio for ministry, promotional, archival, or related purposes, without compensation.

8. Minors

If I am registering or signing on behalf of a minor (under 18), I confirm that I am that minor's parent or legal guardian, and I agree to all of the terms above on the minor's behalf, including the assumption of risk, release, and indemnification. I agree to supervise the minor throughout the event.

9. Code of Conduct

I agree to conduct myself respectfully, to follow the reasonable directions of event staff and volunteers, and I understand that I may be asked to leave for disruptive or unsafe behavior.

10. Governing Law & Severability

This agreement is governed by the laws of the State of Arkansas. If any part of this agreement is found to be unenforceable, the remaining parts stay in full force and effect.

11. Acknowledgment

I have read this agreement and understand that I am giving up substantial legal rights by agreeing to it. I am signing or agreeing to it freely and voluntarily. I am at least 18 years old, or I am the parent/legal guardian of the minor(s) named below.
