1. Certificate of liability insurance in the amount of $1,000,000 per incident and $500,000 per person with the MNCPPC (2425 Reedie Dr, Wheaton, MD 20902) listed under certificate holder.
2. Maryland State Sales and Use Tax License or proof of taxation.
3. Food Service Facility License issued by the Montgomery County Department of Health and Human Services.
4. Vendor address and contact cell phone number.
These documents are required by the Park Dept in order to get a Park Vendor Permit (this is different from your mobile MoCo license). Olney Civic Fund will pay the $50 fee for you.
If your license or insurance renews before May 3, you may send your current policy in order to meet the above payment deadlines. When your renewal is complete, forward the updated paperwork. The park will NOT ALLOW you to sell on park property without a Park Vendor Permit so please get the paperwork to me for submission asap.
