1. LIABILITY WAIVER & ASSUMPTION OF RISK

I, the undersigned parent/legal guardian, understand that my child will be participating in GWCCC's Black Friday Basketball Skills and Drills, hosted by Coach Daniel Coverdale.

I understand that participation in basketball activities, including skills training, drills, physical exercise, and recreational sports, involves inherent risks, including but not limited to physical injury, falls, collisions, sprains, and other accidents.

I acknowledge that my child's participation is voluntary and that I assume responsibility for the risks associated with participation. I hereby release and hold harmless George Washington Carver Community Center (GWCCC), its board members, employees, volunteers, coaches, program facilitators, and representatives from liability for injuries, accidents, or damages arising from my child's participation, except where prohibited by law.

I understand that GWCCC will make reasonable efforts to provide a safe environment and appropriate supervision during the activity.

2. MEDICAL AUTHORIZATION

In the event of an emergency, I authorize GWCCC staff, volunteers, and program personnel to seek emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am responsible for any medical expenses incurred on behalf of my child.

3. PHOTO & VIDEO RELEASE

I hereby grant permission to George Washington Carver Community Center (GWCCC) and its authorized representatives to photograph, videotape, and/or record my child during the Black Friday Basketball Skills and Drills on November 27, 2026.

I authorize GWCCC to use these photographs, videos, and recordings for promotional, educational, informational, and community outreach purposes, including but not limited to social media, websites, newsletters, flyers, publications, and other media.

I understand that these images and recordings may be used without compensation and that my child's name will not be published without additional permission.

I give consent and permission for my child to be photographed and/or recorded.

4. PARENT/GUARDIAN ACKNOWLEDGMENT & CONSENT

I certify that I am the parent or legal guardian of the child named above. I have read and understand this Liability Waiver and Photo/Video Release Form. I voluntarily agree to its terms and give permission for my child to participate in GWCCC's Black Friday Basketball Skills and Drills.
