&bull; &nbsp;This activity will be supervised by YDA staff, interns, and volunteers.&bull; &nbsp; &nbsp;My child must be able to function independently. Failure to do so will result in my child&rsquo;s removal from the program.&bull; &nbsp; &nbsp;The YDA does not provide any health/hospitalization insurance for my child.&bull; &nbsp; &nbsp;I am responsible for promptly picking up my child at the specified time.This program requires a signed YDA permission slip submitted prior to the start of the program. &zwj;All fees are non-refundable and non-transferable - substitutions are prohibited. NO EXCEPTIONS. YDA will not, under any circumstances, issue cash refunds. A nontransferable YDA credit, less a $10 per person/per activity cancellation fee, will only be granted if a cancellation is received at least one week prior to the activity. NO EXCEPTIONS.
