Promotional - I understand that All Things Theatre Company has the right to audiotape, film, videotape, photograph, and otherwise record, edit, and incorporate my child for commercial, media distribution, and/ or any other promotional purposes. I waive any right that I or my child may have to inspect or approve any advertising or other copy that may be used and/or the use that the promotions may be applied. I waive any rights to privacy or publicity that I or my child may have with regard to the promotions and any reuse of the promotions. I understand that All Things Theatre Company has the sole ownership of the promotions in perpetuity and that the promotions may be aired one or more times over any television station, cable system or any other media now known or hereafter developed.*

Liability Release - In consideration of the benefits of instruction provided by All Things Theatre Company, I, intending to be legally bound, do hereby enroll the student(s) named above in the program and do hereby waive claim and release finally the All Things Theatre Company, Jacob Justice, its staff, instructors, agents and other personnel from claim or liability for any injury or accident occurring or arising from the instructional program or incidental sponsored activities either on or off premises. I understand that participation in any instructional and/or recreational activities at All Things Theatre Company is voluntary, and that all Programs and the use of the related facilities and equipment therein carries some physical risk. I understand that if I or my minor child is injured or our property is damaged while participating in the Programs, that the injury or loss will not be covered or reimbursable by All Things Theatre Company. I agree to assume the risk of any and all illness or injury (minor, serious or catastrophic in nature, including claims and suits at law or in equity for any injury, fatal or otherwise) or damage (to person or property) resulting from my/our minor child's participation in all Programs, including the use of the facilities and/or equipment associated with the Program ("Damages"). I hereby waive all claims, on behalf of myself/ourselves and claims by my/our minor child (including claims which may be brought after attaining majority), now or in the future, for any such Damages and do hereby release and discharge All Things Theatre Company and its respective officers, directors, instructors, agents, employees and assigns from any and all liability for any such Damages. I assume full responsibility for all liability in connection with such Damages, and agree to indemnify All Things Theatre Company against any and all such claims and related costs, including claims by my/our minor child that may be brought after attaining majority. I certify that the participant is in good health and that he/she has no physical limitations which would preclude him/her from the safe use of the facilities and equipment related to the Programs offered by All Things Theatre Company and that I/we have sufficient health, accident and liability insurance to cover any Damages that may result as a result of my/our child participating in the Programs, and if I/we have no such insurance, I/we certify that I/we am/are capable of personally paying for any and all such Damages.*

Attendance - In the event that your student is sick, no makeup class will be offered by the class and no refunds will be given. In addition, failure to attend may result in roles being recast for performances. These changes are at the discretion of the director, Jacob Justice, and his staff. Extenuating circumstances will be considered on a case by case basis.*

Medical Release - I authorize emergency first aid care by All Things Theatre Company in the event the student(s) become injured or ill during instructional program or incidental sponsored activities either on or off the premises. If the emergency contact is not immediately available by the phone number provided, I further authorize All Things Theatre Company, Jacob Justice, or such agents as he may authorize, to retain the services of a doctor or other competent medical person in order to treat the injury as needed.*
