READ ME: Troop 8025 Activity Permission Form&nbsp;The Troop Leader is:&nbsp; Suzanne Heino &nbsp;and can be reached at: 330-261-3127 (Keep in mind, we often go to places with limited cell phone coverage or are participating in activities where contact is limited so response time may be delayed)&nbsp; Arrangements:&nbsp;Refer to the GroupMe chat and calendar for drop off and pick up arrangements.&nbsp; Permissions:By completing this registration, I give permission to to the Scout Leaders and adults attending to transport my Scout to / from the activity; otherwise, I will be available to pick her up at the designated time and site. It is the Scout&rsquo;s responsibility to show up to the activity properly prepared for outdoor conditions, one season colder, as discussed at the meeting prior to the event. Failure to do so, will result in not being able to go. If equipment needs to be borrowed, the Scout needs to let the Troop Leader on this form know two weeks in advance so that arrangements can be made.&nbsp; The Troop Leader will notify the parent or guardian at the number you have listed below in the event that there is a change of schedule or emergency.
