It must be accurately and fully completed and signed.</li><li fr-original-style="" style="box-sizing: inherit;">Case Management Fee of $25.00 must be paid before processing begins. Email the proof of payment with unique transaction ID if needed. The fee is not refundable and applies towards the cost of process your paperwork and uploading your records. We will charge additional units for excessive calling, case management etc.</li><li fr-original-style="" style="box-sizing: inherit;">Therapy Compliance: You must attend psychotherapy sessions at least every two weeks or twice a month.</li><li fr-original-style="" style="box-sizing: inherit;">Psychiatric Evaluation and Medication Management: You must be actively seeing your provider (e.g., Psychiatrist) every two months or as ordered.</li><li fr-original-style="" style="box-sizing: inherit;">Assessments: Given your FMLA is based on your medical need we are required by law to provide a objective assessment to support your claim. Past due balance &ldquo;put administrative holds on your records&rdquo; therefore preventing office to complete any requests.</li></ol>&nbsp;Important Reminders<ul fr-original-style="list-style-type: undefined;margin-left:undefined;" style="list-style-type: undefined; box-sizing: inherit;"><li fr-original-style="" style="box-sizing: inherit;">FMLA and MLA are based on your medical or mental health needs and are not guaranteed at each recertification if you have unpaid balance, no shows, cancellations or did not have active case in last 90 days.&nbsp;</li><li fr-original-style="" style="box-sizing: inherit;">For extended FMLA, send us the &ldquo;APPROVAL&rdquo; of the FMLA claim for us to adjust the dates for continuity of care.</li><li fr-original-style="" style="box-sizing: inherit;">FMLA certifications are valid for three months.&nbsp;</li><li fr-original-style="" style="box-sizing: inherit;">To be considered for renewal, all requirements listed above must be completed. Please ensure your updated documentation is submitted at least two weeks.</li><li fr-original-style="" style="box-sizing: inherit;">You must cancel or reschedule your appointments 48 hours prior.
