KRISPY KREME OPT IN

This form is for those who want to OPT IN to Hollywood Vibe Nationals 2025 Krispy Kreme fundraising event hosted by the CC & Co. Parent Boosters Club. Our goal is to raise money for EACH dancer that chooses to OPT IN for the fundraising event. The total dollar amount raised will be split amongst those who have chosen to OPT in and will be paid directly to your dancer's CC&CO account.  IMPORTANT - By choosing to "OPT IN" for the Krispy Kreme event, we ask that all dancers commit to raising their "fair share" of 15 dozen donuts or the equivalent of $105. If your dancer does not reach the fundraising goal, we ask that you donate the remainder of the amount to the booster club so that it can be split amongst the dancers who have chosen to OPT in (remember any donation is tax deductible).
