<div><div><div>Welcome!  LOTWA 🍕 Pizza Mondays</div></div></div><div><div><div> HOW TO FILL OUT THIS FORM 🍕 Please fill out a separate form for each of your students. 🍕This year, you can order for the full year, half year, or by month: <div>Full Year - order by 9/25 by 7pm</div><div>Half Year, Oct-Jan order by 9/25 by 7pm</div><div>Half Year, Feb-Jun - will open in December</div> <div>By-the-Month - always available, orders due by the 25th by 7pm of the month for the next month </div> There is no Pizza Monday during school holidays. The dates pizza will be delivered are listed for each month. FREQUENTLY ASKED QUESTIONS 🍕 School was canceled/my child was out sick - can I get a refund? No refunds are given due to school closure or your child being absent.  🍕 I forgot to buy pizza by the deadline/want to change my existing order- can you open the form for me? Unfortunately, no. The system automatically shuts off ordering at 7 pm on the 25th of each month.  🍕 Is this a fundraiser? Yes! Every purchase you make goes to the LOTWA Foundation general fund, which supports added-value programs like field trip student scholarships, staff appreciation, and free Parent League activities. 🍕 Where do you buy pizza from? We buy from Hungry Howie's. <a href="http://www.hungryhowies.com/" target="_blank">www.hungryhowies.com</a> 🍕 Do I have to pay a credit card processing fee? We use Zeffy for free. At checkout, Zeffy will ask you to donate to cover processing fees. THIS IS OPTIONAL but appreciated! Zeffy uses Stripe as its payment processor.  🍕 Can I drop off cash or a check to Miss Laura at the office? No. This program is run by the LOTWA Parent League, and Miss Laura cannot take money or orders.  🍕 How can I view or confirm my order? A receipt will be emailed to the email address you provided. Once you purchase, your child's name is added to our order spreadsheet, and your order will be delivered to them each Monday of the month you've paid for. No need for you to do anything else!
