This is the order form for the pre-designed remembrance shirt for the event! All shirts will be available for pickup at the event. They will not be shipped. Order cut-off is 7/10/25. A space is left on this design so you may add your loved one's name(s) onto the shirt at our shirt station!  All proceeds from shirts will be split between the UH Parent Bereavement Program and A Touch of Teddy (nonprofit).
