SL Prom Ticket Purchase

Get ready to be part of the SL Class of 2026 Junior Prom. You can buy a single ticket or a pair to attend the event. Please include the First and Last names of attendees. Your ticket purchase includes Dinner, a DJ, a Photo Booth, and a Soda Bar. &nbsp;&nbsp; If you have a special dietary request, please email it to <a data-fr-linked="true" fr-original-style="" href="mailto:khpjuniorprom@gmail.com" rel="noopener noreferrer" style="text-decoration-color: initial; user-select: auto; box-sizing: inherit;" target="_blank">khpjuniorprom@gmail.com</a>. KHP requires that all ticket purchasers and attendees review and send back the acknowledged waiver form they will receive by email after purchasing tickets.&nbsp;

Each attendee of the Kingston Halifax Plympton Class of 2026 Junior Prom must indicate agreement and sign a release form in order to attend (hereafter referred to as &ldquo;Junior Prom&rdquo;). &nbsp;Please scan and return the agreement to khpjuniorprominc@gmail or have the night of the event. If you are bringing a guest, they must also sign this agreement and fill out their own form to indicate agreement to abide by these guidelines. &nbsp;All High School Students and Guests must also have a Parent or Legal Guardian read this agreement and the Code of Conduct, and sign his/her child&#39;s form. &nbsp;General Agreement and Release Form&nbsp;Family agrees to hold harmless, release, waive, discharge, and covenant not to sue anyone involved in the planning or implementing of the Junior Prom from all liability to Family for any loss of damage including but not limited to injury to or death of a person or loss of or damage to their property or belongings while attending the Junior Prom on the Junior Prom premises (Indian Pond Country Club, 60 Country Club Way, Kingston, MA 02364).&nbsp;Family agrees to indemnify releases from any loss, liability, damage, or cost releases that may occur due to the presence of Family in or on the premises of the Junior Prom&nbsp;Family assumes full responsibility for any risk or bodily injury, death, or property damage due to the negligence of releases or otherwise while in or on the premises of the Junior Prom premises and/or while working, or for any purpose participating in the activities of the Junior Formal. &nbsp;&nbsp;Family expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Massachusetts and that if any portion of the agreement is held invalid, it is agreed that the balance shall notwithstanding continue in full legal force and effect.&nbsp;In consideration of being permitted to attend and participate in all Junior Prom activities, Family, Family heirs, executors, administrators, and assigns release and forever hold harmless all releases and their heirs, administrators, executors and assigns from any and every claim, demand, action or right of action whatsoever kind of nature, either in law or equity arising from or by reason of any bodily injury or personal injuries, known or unknown, death, or property damage resulting from any accident which may occur as a result of participation in any and all Junior Prom activities whether by negligence or not.&nbsp;By checking/signing agreement, Family states that both the father and mother of the Family (or legal guardians) have carefully read the above release, know the contents of the release, and sign this release voluntarily.&nbsp;Family releases and holds harmless all officials and professional personnel from any claims whatsoever arising from any and all medical treatment or services rendered to any Family member during attendance and/or participation in any and all Junior Prom activities.&nbsp;This agreement shall be binding on each member of the Family, their heirs, next-of-kin, personal representatives, and assigns. &nbsp;This release contains the entire agreement between all parties to this agreement, and the terms of this release are contractually and legally binding and not a mere recital. &nbsp;&nbsp;By checking/signing, all attendees, students, and parents, agree that they have read, understood, and agree to abide by the Code of Conduct, and that the failure to do so will result in expulsion from the Junior Prom by the designated Police Detail. &nbsp;&nbsp;Attendee Signature________________________________ Parent/Guardina Signature__________________________&nbsp;&nbsp;Code of Conduct&nbsp;The following are rules of expected behavior and etiquette. &nbsp;Failure to comply may result in expulsion from the Junior Prom. &nbsp;&nbsp;No outside food or drinks allowed in Indian Pond Country Club.&nbsp;No smoking or vaping allowed at Indian Pond Country Club either inside or outside the facility. &nbsp;&nbsp;All attendees and guests must arrive between 6 pm and 7 pm and remain inside Indian Pond Country Club until the end of the Junior Prom (10 pm), unless directly excused by a parent or guardian in person. No phone calls will be honored as a dismissal plan.&nbsp;Fighting, destruction of property or any bad behavior will not be tolerated. &nbsp; If any property is destroyed, attendees involved and their family will be held responsible and will be required to cover the expenses incurred. &nbsp;&nbsp;If you are dismissed by a parent or guardian in person, &nbsp;during the Junior Prom, you will not be allowed to return to the Junior Prom upon leaving the premises. &nbsp;&nbsp;Chaperones and local police have the right to call any parent or guardian and request any attendee to leave the event at any time for any reason. &nbsp;&nbsp;No Junior Prom attendees over the age of 19 and no attendees in 8th grade or younger are allowed to attend the Junior Prom. &nbsp;&nbsp;Attendees of the Junior Prom must be in the SLRHS Class of 2026 or a guest of a member of the SLRHS Class of 2026. &nbsp;&nbsp;&nbsp;&nbsp;
