Media Release Acknowledgement and Consent Agreement Statement Through my membership transaction, I hereby authorize and give permission to the Southern Delaware Chorale (SDC), and/or its affiliates, faculties, agents, subcontractors, employees, and members permission to photograph, film, audio and/or videotape images of myself (or my child) using and disclosing my name for the purpose of public advocacy, publication, promotion, outreach, media relations. social media, and/or vocal education and/research. I understand that I may be identified in any use of the above materials. I realize that I will not be compensated in any way for the use of photographs, films, audio and/or videotapes or the publishing thereof. I understand that I am not required to sign this authorization. My enrollment or membership is not contingent on the signing of this form. I accept responsibility for physically excluding myself from photos and recordings, and will hold harmless the Southern Delaware Chorale for any inclusion of my image, or my child&#39;s image.I understand that unless properly revoked, this authorization has no expiration date.
