Participant Agreements

By registering for a Chesapeake Bay Wellness Experience hosted by Evolve Community Foundation, Inc. (ECFINC), you acknowledge and agree to the following:

Code of Conduct

Participants are expected to treat others, staff, volunteers, wildlife, and the environment with respect. Please follow all instructions from program leaders and remain with the group throughout the experience.

Unsafe, disruptive, discriminatory, threatening, violent, or illegal behavior—including alcohol, illegal drugs, weapons, or harassment—is prohibited. ECFINC reserves the right to remove any participant whose behavior compromises safety or the experience of others.

Child Supervision

Participants under 18 must be accompanied and supervised by a parent, legal guardian, or authorized adult at all times unless otherwise stated in writing by ECFINC.

ECFINC does not provide childcare or one-on-one supervision. Supervising adults remain responsible for the minor’s safety, behavior, and medical needs.

Assumption of Risk

Outdoor activities involve inherent risks, including uneven terrain, changing weather, wildlife, insects, physical exertion, slips, falls, dehydration, transportation hazards, and other unforeseen conditions that could result in injury or illness.

By participating, you acknowledge these risks, confirm that you are physically able to participate, and agree to follow all safety instructions.

Liability Waiver

Participation is voluntary. By registering, you release and hold harmless Evolve Community Foundation, Inc., its officers, board members, employees, volunteers, partners, sponsors, and property owners from liability for injuries, damages, or losses arising from participation, except where prohibited by law.

Participants are responsible for their own medical expenses, transportation, and personal belongings.

Photo & Media Release

During the event, photos and videos may be taken for educational, promotional, fundraising, and outreach purposes.

By registering, you grant ECFINC permission to use your image or likeness unless you notify us in writing before the event. Choosing not to grant permission will not affect your participation, and we will make reasonable efforts to honor your request.

Emergency Authorization

If an emergency occurs, you authorize ECFINC to contact emergency services, provide reasonable first aid within staff training, notify your emergency contact, and assist emergency responders as needed.

You understand that ECFINC cannot guarantee emergency response times and that you are responsible for any related medical or transportation costs.

This is for Family Registration for up to 4 participants.
