Through hands-on, animal-assisted experiences with Slater Family Farms small farm animals, the program fosters a true sense of belonging while offering meaningful opportunities for connection, growth, and joy. Each week offers something new&mdash;from leadership and real farm work to creative connection and a final week centered on purpose and celebration. Join us for a 4-week experience designed to build connection and growth over time. Acknowledgment of Risk:In consideration of the risk of injury while participating in an event at Hope Valley (hereinafter referred to as the &quot;Activity&quot;), and in consideration of my desire to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives (collectively referred to as &quot;Releasor,&quot; &quot;I,&quot; or &quot;me&quot;), knowingly and voluntarily enter into this Waiver and Release of Liability. &nbsp; 2. Assumption of Risk:I acknowledge that I am voluntarily participating in the Activity and that I am participating entirely at my own risk. I am aware of the risks associated with participating in this Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others&#39; negligence, conditions related to travel to and from the Activity, or from the conditions at the Activity location(s). Medical Treatment:I acknowledge that Hope Valley is not responsible for any errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of Hope Valley. In the event that I require medical care or treatment, I authorize Hope Valley to provide all emergency medical care deemed necessary, including but not limited to, first aid, CPR, the use of AEDs, emergency medical transport, and sharing of medical information with medical personnel. I agree to assume all costs involved and agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. &nbsp; 4. Fitness to Participate:I acknowledge that this Activity may involve a test of a person&#39;s mental and physical limits and may carry with it the potential for death, serious injury, and property loss. I agree not to participate in the Activity unless I am medically able and properly trained, and I agree to abide by the decision of Hope Valley, Salter Family Farm, official, or agent, regarding my approval to participate in the Activity. &nbsp; 5. Facility Rules:I understand that&nbsp;Hope Valley is a Non-Smoking and Weapons-Free facility, with weapons prohibited unless specifically authorized by written contract. Additionally, all service animals must be approved prior to the activity, and any and all certifications must be presented at the time of contract. Hope Valley disclaims any and all liability arising from incidents involving animals. 6. Photo Release:I hereby grant permission to Hope Valley to capture and use photographs and/or video recordings of me and/or my minor child(ren) during the activity for promotional, educational, and any other lawful purposes. I understand that these images may be used in print and digital media formats, including but not limited to brochures, websites, and social media platforms. I acknowledge that all photographs and recordings taken by Hope Valley are the exclusive property of Hope Valley and cannot be shared or distributed without prior approval from Hope Valley administration. I understand that it is my responsibility, or the responsibility of the activity host or facilitator, to inform Hope Valley in writing prior to the activity if I or any minor participant under my guardianship should not be photographed or recorded. Failure to provide such notification will be considered as consent to be photographed and recorded. In the event that I am signing on behalf of a minor, I certify that I am the parent, legal guardian, or person with a minor(s) in my care, custody and control and have the authority to grant this release on behalf of the minor. 7. Governing Law:I agree that this release shall be governed for all purposes by Ohio Law, without regard to any conflict of law principles. This Release supersedes any and all previous oral or written promises or other agreements. &nbsp; 8. Duration of Agreement:This Waiver and Release of Liability shall remain in effect for the duration of my participation in the Activity, during this initial and all subsequent events of participation.&nbsp; 9. Release of Liability:I hereby acknowledge that I have carefully read this &quot;Waiver and Release&quot; and fully understand that it is a release of liability. To the extent that statute or case law does not prohibit releases for ordinary negligence, this release is also for such negligence on the part of Hope Valley and its employees and volunteers. 10. Understanding and Consent:This agreement was entered into without duress or coercion and is to be interpreted as an agreement between two parties of equal bargaining strength. Both participant and Hope Valley agree this agreement is clear and unambiguous as to its terms, and that no other evidence shall be used or admitted to alter or explain the terms of this agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into. &nbsp; 11. Age and Authority:I, the undersigned participant, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content, and that this release cannot be modified orally. I am aware that this is a contract and that I am signing it of my own free will. &nbsp; 12. Parental Consent:In the event that the participant is under the age of 18, this release must be agreed to by the parent or guardian. I hereby certify that I am the parent or guardian and give my consent without reservation to the foregoing on behalf of this individual.
