Family Membership & Student Participation Agreement
Purpose of Veritas Collective
Veritas Collective provides a shared educational facility and community where independent instructors offer classes,co-ops, tutoring, and enrichment opportunities. Classes at Veritas are taught from a Biblical Worldview.
Independent Instructor Relationship
Families understand and agree that:
- All classes are taught by independent instructors, not employees of Veritas Collective
- Each instructor operates their own program, curriculum, and policies
- Tuition, refunds, and class expectations are set by the instructor
- Payment for classes is made directly to the instructor unless otherwise specified
Any concerns about instruction, curriculum, or class-specific issues should be addressed directly with the instructor.
Annual Family Membership
All participating families are required to maintain an active Veritas membership.
Membership Fee: $100 per family, per year
Membership provides:
- Access to register for classes held at Veritas
- Access to Veritas facilities during scheduled class times
- Inclusion in Veritas communication and program announcements
- Library membership
- Early access to event tickets
- Discount for specific events
- Open gym events,
- Room use
Membership fees are non-refundable.
4. Facility Expectations
Families and students agree to:
- Treat the building, classrooms, and equipment with care
- Clean up after themselves
- Remain only in authorized areas
- Supervise children who are not actively in a class
- Follow all posted building rules and safety procedures
Families are financially responsible for any damage caused by their student.
5. Arrival, Dismissal & Supervision
- Students should arrive no earlier than 10 minutes before class unless otherwise arranged
- Parents/guardians are responsible for timely pickup
- Veritas does not provide childcare or supervision outside scheduled class times
- Students under 10 years old must be escorted in and out unless written permission is provided
6. Health & Safety
Parents agree to:
- Keep students home if they are sick or contagious
- Inform instructors of relevant medical needs or allergies
- Provide emergency contact information
In case of emergency, Veritas staff may seek medical care if a parent cannot be reached.
7. Liability Waiver
By participating, families acknowledge:
- Participation in classes and activities involves inherent risks
- Veritas Collective provides facility space only and is not responsible for instructional activities conducted by independent instructors
Parent/Guardian agrees to release and hold harmless Veritas Collective, Six Forty Corp, its staff, volunteers, and facility partners from claims related to participation, except in cases of gross negligence.
8. Communication
Families agree to:
- Read Veritas emails and announcements
- Communicate directly with instructors regarding class-specific questions
- Keep contact information current
9. Code of Conduct
Veritas Collective is a community environment. Families agree to:
- Treat instructors, staff, and other families respectfully
- Address concerns calmly and constructively
- Follow all behavioral expectations
Veritas reserves the right to revoke membership or facility access for disruptive, unsafe, or inappropriate behavior. Membership fees are non-refundable.
10. Photo & Media Release I give permission for Veritas Collective to use photos of my student for promotional or community purposes.
11. Agreement
Purchasing membership acknowledges agreement to this contract.
