Waiver Of Liability
Participation in our TAJ HOOPS ENTERPRISES INC gym events and activities requires an acceptance of an annual waiver of liability. It is included automatically as a part of your membership application, but non-members are still required to accept this waiver of liability to participate.
RELEASE OF LIABILITIES
TAJ HOOPS ENTERPRISES INC are permitting and allowing me to participate in activities at the HOOP PHENE ELITE facility
For and in consideration of the foregoing, I hereby release and forever discharge and promise not to hold liable TAJ HOOPS ENTERPRISES INC, respective officers, and Board Members from any action, suits, damages, claims or judgments that may result from any personal injury or loss of property.
This release is binding to me and my heirs and personal representatives and is valid for all TAJ HOOPS ENTERPRISES INC activities that I participate in during the year.
