Only pay this fee AFTER your skater has passed their test according to their test packet/protocol. This fee is only paid by FSCPC Members, to process the test submission to USFSA.

Please complete the form and email it to L-Jwilliams@comcast.net.  There is a $10 fee for the Club’s completion/return of the form to you.
