Evergreen Catholic School Division will be hosting an open house to provide families with information and resources about Transitions Beyond High School for students with disabilities.&nbsp;Additionally, caregivers will learn the importance of Family Support for Children with Disabilities (FSCD) service agreements and why families should use the service so support continues beyond school years, such as PDD. Each family will receive a resource binder with a transitional planning checklist to guide caregivers through the transition from childhood (Children&#39;s Services) to adulthood (Adult Services). Families can do the necessary paperwork with the aid of the checklist. A resource manual with local resources and assistance has been created by Evergreen Catholic School that will be provided as well.We have invited agencies that can help answer questions or concerns that you may have.&nbsp; In partnership with Gateway and Evergreen Catholic Schools.
