Dear Parents and Student Athletes, Please complete the SVMS Sports Registration form and select the sport your child would like to participate in. To participate parents/guardian and athlete must agree upon the Code of Conduct when filling out the Sport Registration Form. After school sports are very important to the growth and development of many students. Lessons learned from participation in school athletics are priceless! These include learning new skills, sportsmanship, discipline, camaraderie, school spirit and a sense of belonging. *A signed VOLUNTARY ACTIVITIES PARTICIPATION FORM must be on file with the district before a student will be allowed to participate in extracurricular activities, tryouts or open gym opportunities. Our after-school athletic program relies on the support of our families. It costs approximately $125 per student-athlete to operate each team. Donations help cover coaches' stipends, officials' fees, tournament costs, equipment, uniforms, and other program expenses. Instead of multiple fundraisers, we ask families to consider making a tax-deductible donation to support our award-winning athletic program. Your generosity helps provide a positive experience for all of our student-athletes. Thank you for your support!
