Please submit this form by September 18th. Just like last year, we're asking you to opt in or out of all five lunches as a package. Please select the quantity of children you are registering for Fun Lunch and then fill out the form with their selections below. You have the option to opt out of all lunches should you need to in the form but we ask that every student have a form filled out.

If you are able to contribute, suggested donation is $40 per student.
