Consent and Authorization:By completing the registration, I, the undersigned/buyer, [Responsible Adult age 18+], hereby grant permission for the named student to participate in the upcoming bus trip to and from Hampton University scheduled to take place on November 24, 2026 and returning November 29, 2026.Health Information: I understand that the chaperones will take necessary precautions to ensure the safety and well-being of the students during the trip.&nbsp;Emergency Medical Treatment: In the event of an emergency, I authorize the chaperones to seek medical treatment for the student. I understand that every effort will be made to contact the listed me and the listed emergency contact in case of a medical emergency.Supervision and Behavior: I acknowledge that the student will be under the supervision of alumni chaperones during the trip. I expect the student to adhere to the code of conduct and behavioral expectations.Communication During the Trip: I will provide reachable contact numbers during the trip, and I understand that the chaperones may contact me in case of any important updates or emergencies.Transportation: I acknowledge that the student will travel via coach bus and is expected to adhere to the meeting time schedules. Baltimore NHAA is not responsible for lateness at either end.Cost and Payment: I understand the cost associated with the trip, which includes transportation and snacks.
