The George Washington Carver Community Center (“GWCCC”) is hosting the Pitching for Unity Softball Fundraiser. This waiver applies to any individual participating in, attending, volunteering at, providing services for, or otherwise affiliated with the event, including participants, community members, volunteers, vendors, community service participants, partners, and individuals affiliated with participating organizations.

By signing this document, I acknowledge that I have read, understand, and voluntarily agree to the terms below.

1. ASSUMPTION OF RISK

I understand that participation in or attendance at a softball and community fundraising event involves inherent and foreseeable risks. These risks may include, but are not limited to, falls, collisions, being struck by a ball or equipment, physical exertion, sprains, strains, fractures, illness, weather-related conditions, and other injuries or medical conditions that may arise during or in connection with the event.

I voluntarily assume these risks and accept responsibility for my participation and conduct during the event.

2. RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by applicable law, I release and agree not to hold responsible the George Washington Carver Community Center (GWCCC), its officers, directors, employees, volunteers, representatives, agents, successors, assigns, and participating event partners for claims arising from my participation in or attendance at the Pitching for Unity Softball Fundraiser, except to the extent such claims cannot lawfully be waived or released.

I understand that GWCCC and the event organizers cannot guarantee that injuries, illnesses, accidents, or other incidents will not occur.

I agree that I am responsible for determining whether I am physically able and appropriately prepared to participate in the event.

3. MEDICAL TREATMENT

In the event of an emergency, accident, injury, or illness, I authorize event personnel to obtain reasonable emergency medical assistance for me when necessary. I understand that I am responsible for my own medical expenses and that GWCCC does not provide medical or accident insurance for event participants.

I understand that any applicable health insurance coverage remains my responsibility.

4. PERSONAL PROPERTY

I understand that I am responsible for my personal belongings and valuables while attending or participating in the event.

To the fullest extent permitted by law, GWCCC and the event organizers are not responsible for personal property that is lost, misplaced, damaged, or stolen, except where responsibility cannot lawfully be excluded.

I agree to keep my personal belongings secure and within my possession or reasonable sight.

5. PHOTO, VIDEO, NAME & LIKENESS RELEASE

I understand that GWCCC and/or its authorized representatives may photograph, videotape, or otherwise record participants and attendees during the Pitching for Unity Softball Fundraiser.

By signing this form, I grant GWCCC permission to use my photograph, video image, voice, name, likeness, and/or statements in connection with the event and GWCCC’s educational, promotional, fundraising, community outreach, social media, website, print, and other communications.

I understand that these materials may be published or distributed in various forms of media, including websites and social media platforms, without compensation to me.

I waive any right to inspect or approve the finished materials before they are published, to the extent permitted by law.

6. VOLUNTARY AGREEMENT

I confirm that I have had the opportunity to read this document carefully and ask questions before signing it.

I understand that by signing below, I am voluntarily agreeing to the Assumption of Risk, Release and Waiver of Liability, Medical Treatment provisions, Personal Property Acknowledgment, and Photo/Video Release contained in this document.
