This membership is intended for direct employees. By registering for this membership, you agree to a bi-annual payroll deduction of $20 (total of $40 per year).

Statement of Conditions

1. Member acknowledges that they have been informed of the Club’s present membership dues before signing this authorization form.
2. Member signs this form voluntarily.
3. Member understands that membership dues are subject to review and may be changed by action of the Club’s Board of Directors.
4. FCA US LLC and its subsidiaries make no representations concerning the operation or financial condition of the Chrysler Management Club and assume no obligation or liability of any nature to anyone in connection with the activity of the club.
5. For direct employees: deductions will be taken in the first pay ending period in February and August.
Retiree deductions will be taken in September.
6. For contractors, retirees without pension, non-US employees, interns, and co-ops: payment is due upon submission of this form.

This membership is intended for contract employees or employees who do not receive payroll from FCA US (such as FCA Canada). By registering for this membership, you agree to make an annual dues payment of $40 through Zeffy to maintain your membership status.
