Cornerstone
Transitional group home providing rehabilitation services to adults with serious mental illness. Referrals through Community Services Network (CSN).
Park Street Inn
Intensive residential program offering comprehensive care to those transitioning from psychiatric hospitals. Referrals via DMHAS Office of the Commissioner.
Park Street Residence
Transitional program providing case management, crisis intervention, and symptom management for individuals with mental illness and co-occurring disorders. Referrals through CSN.
Sierra Center – Pre-Trial/DMHAS
Congregate care setting for pre-trial adult males with mental health diagnoses, providing case management and skill-building services for community re-entry. Referrals via Court Support Services Division and DMHAS Jail Diversion Program.
Center for Behavioral Health
Offers innovative substance use and mental health treatment services for adults and families, specializing in trauma, PTSD, substance use disorders, anxiety, and depression.
