Provides education, outreach, and application assistance for Florida KidCare, offering quality, affordable health and dental insurance for children.
Health Literacy Education
Offers community-based health literacy education using an award-winning curriculum to help vulnerable populations gain essential health information and decision-making skills.
Care Coordination Panels
Coordinates tailored care panels in rural communities to connect residents with available services, including healthcare, social services, and insurance enrollment support.
Professional Development & Training
Coordinates an annual Rural Health Regional Symposium and offers other trainings, including a Health Literacy Summit, with CEUs available for some activities.
Chronic Disease Self-Management Program
Provides free six-week workshops for rural residents to learn self-management skills for chronic conditions, promoting healthful activities and emotional well-being.
Diabetes Self-Management Program
Offers free six-week workshops focused specifically on diabetes self-management, teaching monitoring, complication avoidance, and stress management skills.
Health Insurance Enrollment Assistance
Offers free assistance from trained Navigators to help individuals understand and enroll in health coverage options through the Marketplace, Medicaid, KidCare, and Medicare.
Prescription Assistance Program Enrollment
Assists individuals in identifying and applying for Prescription Assistance Programs to access affordable medications through various resources.
