The PHC Foundation, established in 2012 by Professional Health Care Home Health and Hospice, supports local patients and families by providing resources to overcome life's obstacles. This community-based 501(c)3 organization affirms life and strives to respect the uniqueness of each individual, meeting their physical, emotional, and spiritual needs with love and compassion, particularly for the indigent and under-insured. The foundation also assists indigent hospice patients with unmet needs at the end of life.
