Primary Health Care
Offers healthcare to those without a family doctor, or struggling with mental health, substance use, or homelessness. Services include assessments and counseling.
Navigation Services
Helps individuals navigate health, social, and community services. Uses a holistic approach to assess needs and strengths, supporting people in their journey.
Scattered Site Case Management
Provides intensive case management in the community for individuals in private market housing who benefit from wrap-around support.
Peer-Supported Housing
Manages supportive housing buildings with on-site support for tenants facing substance use, mental health issues, and chronic homelessness.
Peer Support
Offers a relationship between people who share a common lived experience. Peer Support Training is provided to those in the community who want to share their knowledge.
Mobile Health Unit
Provides care to individuals in shelters who face barriers to traditional healthcare through a mobile unit.
