Thanks for your interest in Fall Choir! Please read through the following information before filling out this form. Choir is put on by the Green Acres PTO. Participation is first come, first served. If you have more than one child you are signing up, you may add this item to your cart more than once. Students and parents must be committed to attending every week. Please email [email protected] if your child is ill. Due to liability, students will not be allowed to attend until this form has been filled out completely and submitted, AND payment has been received by the PTO. You will receive a confirmation email and receipt when we have received your completed registration. Please do not send your child/ren if you have not received this confirmation.
