STUDENT/PARENT/GUARDIAN CERTIFICATION, STATEMENTS AND AGREEMENTS FORM 7. CAREER AND ABILITY ESSAY OUTLINE IN ADDITION, THE FOLLOWING MUST BE INCLUDED WITH EACH APPLICATION: A. OFFICIAL SCHOOL TRANSCRIPT WITH SAT AND/OR ACT SCORES B. FAFSA SARS REPORT ON EXPECTED FAMILY CONTRIBUTION
