No form is necessary. We do supply grant manuals at a cost of $10 to anyone interested in applying that specify word format for applications and materials that are required. Along with the application they are to provide proof of charity status, an internal review board form and any instruments and references that apply to their application. A resume for each principal investigator is also required.
