THE APPLICANT MUST SUBMIT COPIES OF THE ORGANIZATION'S IRS DETERMINATION LETTER OF 501(C)(3) STATUS AND ANY STATE DOCUMENTATION OF EXEMPT STATUS. APPLICANT SHOULD BE PREPARED TO SUMBIT BUDGET SUMMARIES AND RELATED FINANCIAL RECORDS IF REQUESTED.
